Il est Premier ministre de 1981 à 2003, sous les couleurs de l'Organisation nationale unifiée malaise, membre du Barisan Nasional, au pouvoir de 1957 à 2018.

À la tête de Pakatan Harapan, il redevient Premier ministre après les élections législatives de 2018. Il est alors le chef de l'exécutif le plus âgé au monde, devant le président tunisien, Béji Caïd Essebsi.
